ConsultTed Patient Privacy Policy and GDPR Consent for Pilot
ConsultTed is running a pilot to test a new application. In order for the pilot to run, ConsultTed will need to collect and process some of your personal data. Below is information regarding the pilot and the use of your personal data.
Unlike when the application is fully up and running, for the pilot, your name, address, date of birth, other contact details, gender and details of the GP practice, with whom you are registered, will not be stored by the pharmacy or the application.
ConsultTed is an application owned and operated by Health Tech Horizons Ltd, a company incorporated and registered in Northern Ireland with company registration number NI719472. Its registered office is at The Amp Growth Incubator, 11 Ebrington Square, Londonderry, Northern Ireland, BT47 6FA.
What categories of personal data will ConsultTed process during the pilot?
The following data fields are recorded on the ConsultTed application when you attend a community pharmacy consultation during the pilot:
· Health details – medical history, allergies, current medication, symptoms experienced, potential ailment diagnosis, other relevant health related information to aid diagnosis.
· Treatments - recommended treatments, treatment health products purchased and recommended health specialist referral(s) or signposting.
What will ConsultTed be doing with the personal data collected during the pilot?
The information set out above will be used for pilot data analysis by ConsultTed. This will be to assist with providing a diagnosis and to recommend treatment. It may also be used to signpost you towards other healthcare professionals and/or refer you directly to your GP for treatment and further diagnosis/treatment.
Where will ConsultTed get my personal data from?
Much of the data ConsultTed will use during the pilot will have been provided directly by you when you make or attend a community pharmacy consultation. This will principally be information about your symptoms, what medication you have been taking (if any) and existing treatment you are receiving.
The ConsultTed application will also receive data directly from the pharmacy about health treatment products you have purchased at that pharmacy, if it is a ConsultTed participating community pharmacy.
Will ConsultTed share my personal data with anyone else?
Yes. To help provide the best care or service for you, ConsultTed may need to share your information with:
· Other healthcare bodies and professionals, including GPs. Where a referral to a GP is needed, you will be asked to provide your actual personal contact details and agree to these details being shared with your GP for these purposes.
· The Strategic Planning and Performance Group (SPPG) of the Department of Health (DoH) and Business Services Organisation (BSO) for the purposes of administering and managing health and social care services.
Anonymised information may also be shared with:
· UK Health Security Agency (UKHSA) for the purposes of national disease monitoring.
· Department of Health (DoH) Northern Ireland for the purposes of health monitoring.
· Other community pharmacy stakeholders such as Community Pharmacy Northern Ireland for purposes of analysis of pilot impact and usefulness.
How long do you keep my personal data?
This consultation record (but not your personal identifiable details) will be retained by ConsultTed in line with Department of Health Retention Policy identified in the document ‘Good Management, Good Records’ which can be viewed at department of Health’s Good Management, Good Records (see section M, which outlines the requirements for retention). Neither your personal identifiable information nor the consultation record will be retained by the community pharmacy as part of this pilot.

Does ConsultTed transfer my personal data to other countries?
No, this will not happen during the pilot.
What rights do I have?
· You have the right to obtain confirmation that your data is being processed, and access to your personal data.
· You are entitled to have personal data rectified if it is inaccurate or incomplete.
· You have a right to have personal data erased and to prevent processing, in specific circumstances.
· You have the right to ‘block’ or suppress processing of personal data, in specific circumstances.
· You have the right to data portability, in specific circumstances.
· You have the right to object to the processing, in specific circumstances.
· You have rights in relation to automated decision making and profiling.
